Overview

The v.3.5 account describes an underground aberration with an approximately eight-foot-wide body and a weight around 500 pounds. It speaks Common, stays concealed when neither threatened nor hungry, and uses tentacles to drag prey toward its mouth. A successful hold permits constriction. Its bite transmits filth fever, with an incubation period and losses to Dexterity and Constitution. Those details belong to the older disease system, rather than the hit-point reductions used in later versions.

The 2014 Monster Manual gives the otyugh its own language and one-way telepathy: it can send simple messages and images within 120 feet, but the recipient cannot answer by that channel. Two tentacles can hold separate Medium or smaller captives, who are both grappled and restrained. Tentacle Slam strikes them against each other or a surface and may stun them until the end of the otyugh's next turn. Its bite begins with a disease save; subsequent saves occur every 24 hours, and failures reduce maximum hit points until the disease is cured.

In the revised 2025 block, a bite imposes the Poisoned condition on a hit and the continuing save occurs when the victim finishes a Long Rest. The initial disease save from 2014 is not repeated in that attack. Tentacle hits now specify grappling without the legacy restrained condition, and the slam's stun expires at the start of the otyugh's next turn. One-way telepathy remains explicit. Communication and a neutral alignment provide encounter possibilities beyond treating it as a mindless attacker; they do not supply a complete society or a waste-feeding ecology absent from these inspected public blocks.

This comparison follows the v.3.5 System Reference Document, the legacy 2014 Monster Manual, and the revised Monster Manual published in 2025. Their classifications and action rules describe different editions of the same named D&D creature. The retained 2000 manual is historical bibliography, rather than a newly inspected witness for these differences.

Canon & continuity

The complete dedicated v.3.5 SRD entry was read through its licensed third-party reproduction. The official D&D Beyond legacy and revised stat blocks were separately inspected, including any public description. The revised page identifies Monster Manual page 233. Full commercial manual chapters and campaign-specific encounter histories were not inspected.
